Medication Management Services
In many instances, seniors are taking medications for complex diseases which can have interactions if not taken properly and consistently. It is important that time schedules for taking the medications be followed carefully. As time elapses between the doctors’ office and the pharmacy remembering the medication instructions can be difficult. The pharmacy print out is difficult for lay persons to interpret, understand and actualize. Therefore services assisting with MEDICATION MANAGEMENT can be extremely prudent and provide the safety net that seniors may need to maintain their wellness program.
Medication Management consists of:
1. Needs assessment by a Regestered Nurse to set up the medication management program. This includes a liaison call to the primary physician to re-check orders. A letter will be sent to the physician explaining the program.
2. Pre-pour medications for weekly or biweekly distribution depending on the medications.
3. Contract agreement signed and established.
4. Forms left for the senior to take to doctor’s visit for notification of medication changes.
5. Senior must agree to call nurse if medications have been changed in order to keep the medication box updated.
6. Reordering medication can be assumed if problems have existed.
7. Monthly e-mail sent to family members.
8. Blood pressure and pulse recorded each visit.
9. Continuous medication education as needed.
10. Use of medication sheet will be established.
