What is a Geriatric Care Manager?

According to the National Association of Professional Geriatric Care Managers, a Geriatric Care Manager is a health and human services professional, such as a gerontologist, social worker, counselor, or nurse, with a specialized body of knowledge and experience related to aging and elder care issues.

Ivory House is fortunate to count several Professional Geriatric Care Managers among our staff. In addition to our rigorous standards of care management, these individuals:

  • Are members of a national association committed to the care of older adults
  • Have satisfied professional reviews of their education and experience
  • Adhere to a unified “Standards of Practice” and “Pledge of Ethics”
  • Rely on a network of resources to assist in the care of your family

Some of the most important ways they help are to:

  • Provide crisis intervention and avoid unnecessary hospitalization
  • Assist with moving an older person to/from a retirement/nursing facility
  • Share extensive knowledge of cost, quality and availability of local services
  • Reduce inappropriate institutional care and overuse of services
  • Provide continuity of care management to reduce time, stress and cost
  • Foster a client-centered approach free from bureaucratic “red tape”
  • Act as liaison to distant family members, providing information and counsel
