Ivory House Health Services offers a three level approach of geriatric care management services to meet the needs of seniors and their families.

  1. A RETAINER PROGRAM to be utilized in time of need. We will do a need assessment and act as an advocate for our client, especially allowing adult children who live at a distance to have peace of mind, knowing that a healthcare professional will act as their eyes and ears. Your loved one will also receive a monthly phone call to check if there are any problems. The senior is encouraged to call her assigned manager if she is experiencing difficulty.
  2. A PROFESSIONAL PLAN OF CARE that loved ones can implement. Understanding the strengths of the family, our care manager can guide them through the process.
  3. A PROFESSIONALLY IMPLEMENTED PLAN OF CARE that our care managers coordinate and follow on behalf of the client. Communication is established with family members and dialogue established.

How do I know which level is right for me.?

Ivory house health services will meet with all family and the client to help you understand which level of program will meet the needs of both you and your loved one.
