Hospital to Home ChecklistA precarious juncture along the path to healing and well-being is the transition in care from the hospital to the home environment.  It is not unusual to find confusion about medications, difficulty in getting timely physician follow-up appointments, spaces that are hazardous to navigate, inadequate daily nutrition and irregular oversight of personal care.  As a aid to clarifying hospital directives, Ivory House launched a Transitional Care Checklist to facilitate documenting home health care needs for a patient and for assistance in the home.

Brava! Award for Ivory House CEO

Ivory House congratulates its owner and CEO, Hazel Kuchinsky,on her recognition by SmartCEO with a 2010 Brava!
