Care Management
Care management is a holistic approach centered on the individual, not individual illness or injury. It is the honest endeavor to assess and address all aspects of a person’s welfare.
It is holistic because it involves the whole person and ways to maintain and improve their quality of life. It is also honest, with the understanding that “improvement” doesn’t always mean a return to one’s previous state.
Our care management focuses on realistic outcomes that are both achievable and measurable. Clients and their families know what to expect, and can readily judge the progress we make along the path of improvement.
Services related to care management are primarily consultative and based on hourly fees. A typical plan would include the following elements:
- Initial consultation and assessment
- Development of a comprehensive, individualized plan of care
- Implementation of private duty care as needed
- Detailed reporting and analysis of outcomes relative to plan goals
- Regular monitoring and consultation with family and other caregivers
- Liaison with key professionals
- Advice about alternative care systems available at different stages
- Qualified review of local area resources
- Referrals to other services
- Recommendations on legal and financial matters
- Periodic review and re-evaluation of care management plan
Though specific features of each plan will vary, all clients benefit from a single and central source of information about the status of their care.
