Hazel Kuchinsky

Chief Executive Officer

Hazel Kuchinsky, APRN-BC – Owner and CEO
Hazel Kuchinsky is both owner and chief executive officer of Ivory House Care Management, doing business as Ivory House Health Services. Mrs. Kuchinsky has a significant background that exemplifies her passion for caring for those in need of healthcare assistance in the home, a career consisting of Mental Health consulting, directing Psychiatric units, and developing Medical Geriatric services in General Hospitals as well as private practices.

She graduated with a Bachelor of Science Degree in Nursing and completed a Clinical Specialty in Psychiatry at the University of Pittsburgh. She provided Mental Health Consultation to the Pittsburgh Public Schools where she was instrumental in developing testing for disadvantaged children, developing large groups of volunteers to assist in their education, and in developing a Center for Families in Crisis focusing on divorce, death and dying. Ms. Kuchinsky has had a private practice in psychiatry and as a therapist at a college in Carlisle, Pa.

The last 20 years have been focused on developing caretaking systems for Drug and Alcohol abusers and Geriatric patients. Her investment in Geriatrics began when she started an assisted living facility to care for her severely ill father. The business evolved into a holistic care management model and convinced her that Care Management and Private Duty services were essential components of the healthcare system.

Mrs. Kuchinsky’s outstanding work in modeling care management in the home has made her a fixture among local physicians in the industry. She was honored by Smart CEO Magazine with the Annual Brava! Women Business Achievement Award in 2010.

Nursing Service

Mary Rose Martinelli

Mary Rose Martinelli, APRN-BC – Owner and Vice President
Ms. Martinelli is co-owner and co-founder of Ivory House Care Management. She has a Master’s in Child Psychiatry from the University of Cincinnati and a Masters of Hygiene degree from the School of Public Health at the University of Pittsburgh. Her early career consisted of working for pediatricians and she was instrumental in developing a program for emotionally disturbed children at Western Psychiatric Institute in Pittsburgh. Later she worked in several psychiatric hospitals in the Philadelphia area, where she developed therapeutic programs for in-patients and a program for geropsychiatric patients. She had numerous responsibilities as Director of Nursing, Director of Quality Assurance and Onsite Administrator. During this period, she was a Surveyor for The Joint Commission (JACHO) on Accreditation on Health Organizations and for the Federal Government. She continues to survey psychiatric hospitals for the Centers for Medicare/Medicaid (CMS).

In her position as vice president and geriatric care manager, Ms. Martinelli oversees the administrative functions of the business as well as performing care management services for the elderly. Her interests include reading, traveling and volunteering in the community.

Michele A. Parisi

Michele A. Parisi, RN, BSN, CM/RND – Director of Clinical Services
Michele Parisi is an advanced practice nurse who is a Care Manager Certified. She has extensive management experience within hospice and palliative care, oncology, and emergency/trauma patient care. She is well versed in acute clinical care, in particular. At Seasons Hospice and Palliative Care, she was responsible for the daily operations of the Inpatient Hospice Unit, including discharges, admissions and deaths. At the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins Hospital, she developed and implemented customized nursing documentation for Radiation Oncology, a Gliasite Brain Brachytherapy program, iodine contrast infusion procedures and a nursing and patient care/management model. Her prior work experience was at Sinai Hospital, Johns Hopkins Hospital, University of Rochester/Strong Memorial Hospital and NIH in Bethesda, MD. Ms. Parisi has additional licensure and certification with the American Registry of radiological technologists and for End of Life: Palliative Care. She also is a faculty member at CEU Professor, Inc., an internet CE credit website for CNAs, LPNs and RNs. She is a graduate of the University of Rochester and received subsequent training at Montgomery Hospital and Johns Hopkins University.

Mary T. Schenning

Mary T. Schenning, RN, MEd, ANCC– Care Manager
Mary Shenning is a psychiatric nurse, therapist and a community liaison for the chronically ill. She has extensive clinical experience at various psychiatric hospitals, psychiatric units, and special institutions. In addition to her private practice, she provided specialized clinical psychiatric services at the Greater Baltimore Medical Center, the University of Maryland Medical Systems, Sinai Hospital’s Mobile Treatment and private services for assessments, family therapy, psycho-social evaluations, education and treatment, often addressing addiction and detoxification. She developed a ‘Creative Expressions’ group for feelings through various media and a Drug, Alcohol Education Group for hospital in-patients.

Ms. Shenning is a graduate of the University of Maryland with a Master’s Degree in Human Development and Bon Secours Hospital School of Nursing.

Susan Leidy

Susan Leidy, RN, MSN, CMSRN – Educational Trainer, In-Home Services
Susan Leidy is drawing on her extensive background in medical education in developing educational modules as online training for the Certified Nursing Assistants on Ivory House staff. In prior years, she has served as Clinical Director of Care Management and In-home Services, where she provided care management services, developed and implemented clinical protocols, fostered ongoing development of staff and monitored quality of care.

Prior to joining Ivory House, Ms. Leidy was Director of Nursing Education, Medical Clinical Nurse Specialist and Director of Geriatric Clinical Services during her twenty-two years at Greater Baltimore Medical Center.

Ms. Leidy is a member of the Academy of Medical Surgical Nurses and the national honor society of nursing, Sigma Theta Tau (Eta Chapter.) She received her diploma in nursing from the Washington Hospital Center School of Nursing in Washington, DC, a Bachelor of Science from Elizabethtown College and a Master of Science in Nursing from the University of Pennsylvania. In addition, she holds a Master of Liberal Arts from the Johns Hopkins University. Active in her professional organization, Ms. Leidy was selected as an exam writer by the Academy of Medical Surgical Nursing Certification Board.

Marketing and Sales

Patricia T. Bloh

Patricia T. Bloh, BS – Community Liaison Specialist
Ms. Bloh has a real passion for sales and hospital/physician relationships, in particular. These days, her passion is focused on promoting the outstanding and distinguished home health care provided by Ivory House Health Services.

Ms. Bloh started her medical career as Director of a local medical office specializing in reproductive health. She then moved into the pharmaceutical industry where she spent 17 years working for Wyeth, Roche, Abbott and Otsuka. In this capacity, she became well recognized among hospital staff and office-based physicians throughout the Greater Baltimore area. Ms. Bloh continued her growth in the industry when she expanded into representing durable medical equipment to assist with respiratory problems. Her expertise in relationship-building is based upon trust, follow-through and the frequency of her visitation.

Ms. Bloh holds a BS degree from the University of Delaware. She resides in Annapolis with her two young adult children.

Accounting and Administration

Gale Herr Sarubin

Gale Herr Sarubin, BS – Accounting Manager
Gale Herr Sarubin joined the Ivory House team as Accounting Manager in February 2008 and is responsible for accounts payable, accounts receivable, billing, and payroll. She holds a Bachelors of Science degree in Business Administration. Ms. Sarubin has worked in Accounting and Business Administration for the more than 25 years. She conscientiously assists clients and their families with billing or payment questions in the service mode of Ivory House. A key contributor to financial budgeting and management oversight, she is invaluable for her attention to detail and knowledge of financial systems. She continues her service commitment outside of the office where she is devoted to her two college-age children.

Martina Jackson

Martina Jackson – Administration Manager
Ms. Jackson joined Ivory House Health Services in 2007 and became Administration Manager in 2010. She has had a variety of responsibilities including payroll & billing, marketing, and currently is the on-call staffing coordinator. She specializes in CNA/GNA hiring and employee orientation. Ms. Jackson diligently staffs all of our cases, and acts as a family liaison providing exceptional customer service to all of our Clients. Ms. Jackson has been both a nursing assistant and a geriatric nursing assistant for over 13 years, providing experiential training and experience for her present role.
